I have a non-O retirement visa. I'm going to immigration on December 18th to apply for my one-year extension. I'm also applying for a Multiple Re-entry visa. I'm probably going to Cambodia for a few days sometime in January 2026, and I'll definitely be returning to Belgium for about two months in June. What should I fill in on the TM 8 form? Thank you in advance for your positive responses.

To be honest, it doesn't matter. Just put something in there. A re-entry permit is valid until the end of your extension regardless, and they're not going to ask you for your trip information to verify anything. You can put whichever trip you'd like.
Although given the current situation, might be better if you just put the Belgium trip.
