What should I do if my Non-O visa application is requesting Non-OA documents and how do I handle the departure date for my extension?

Hi, hopefully I can get some assistance with this. I am currently completing my non-O visa application. On the main page where you choose the visa type the only one I could find was listed as Long stay (O-A). I choose that and it shows on the home page as a Non-Immigrant Visa (O). All good until the documents page. It is requesting the documents for the Non-OA such as medical certificate showing no prohibited diseases, Health Insurance, criminal record check etc. These are mandatory fields. Can anyone assist on what I do here?

Also, I intend on applying for the one year extension but it is asking for a departure date. My initial accommodation is 88 days, do I use that or put one year in?
