That’s from the Thai Consulate in Los Angeles website. They are still asking for ASQ hotel information too. It’s confusing, but I’m slowly pressing through.

just write N/A on a piece of paper and upload. They use the same application for every visa type and there are many things that don't apply

any policy with $10,000. Travel insurance is usually cheap and quick to get something to upload.
