Regarding the medical insurance requirement for renewal of retirement visa O-A. If because of pre-existing medical condition and age, if one cannot get coverage, will they have to leave Thailand, even after being here for 15+years? Alternatives?

Sorry but the requirements of Immigration are the set requirements. I believe does not change the situation of not meeting what is asked. Sorry for your friend, but he might ask his friends what visa they used.

If you can not meet the requirements of the Extension of Stay it will be denied, no matter how long you did stay inside Thailand before.
