What documents do I need to provide when lodging an overdue TM30 for my visa in Thailand?

On Monday, I am going to CW to lodge my overdue TM30. I know there will be a 800Baht fine, that's Ok. I have the Lease agreement from the owner with a signed copy of their National ID card. Apart from copies of my passport pages, what else do I need to provide? Thanks
