Depends how you entered. With a tourist visa (60 days) you go to Chaengwattana - counter J, visa exempt (45 days) you go to IT Square Laksi Plaza 3rd floor, counter K.

Copy of your visa - either the sticker in your passport or pdf evisa (the former signed by you). * if you had one.
Copy of TM6 departure card (signed by you) *if you were issued one - if you entered by air after July ‘22 you wouldn’t have been given one so leave the space on the form blank.

Some offices may ask for a TM30, which your hotel or landlord should have filed on your behalf. A screenshot of their filing it online will generally suffice as evidence you have one - except if you use the Chiang Mai office. You have to get the screenshot converted to the paper receipt from a normal TM30 filing.
I entered on a 60 days sticker visa. Which of the immigration centre is unlikely to ask for the TM30 because the hotel I'm currently staying at didn't file for it. And my visa will expire on the 10th of April. Is it still possible to ask the hotel to file for TM30 ? Thank you for your info.
if a TM30 is not on file in your records in their database, you will be asked to provide one, and not issued your extension until you do. If it is in the system already they will not ask you to see it. I witnessed four people at Chiang Wattana refused their extensions until they could produce a TM30 in their name. Your hotel was legally supposed to file one within 24 hours after you started staying with them. This is a big problem with staying at Airbnbs
on a 60 day tourist visa you must go to Chaengwattana counter J. The hotel can file a TM30 for you today. Unless you have an extensive history of entries to Thailand you probably won’t be asked for it there.
