What documents do I need to change from visa exempt to a Non-O visa in Thailand?

I am preparing my documents to change from a visa exempt to a Non-O based on over 50 and looking at requirements as per attached form i got at Chiang Mai immigration. I went for a resident certificate but the lady who isdues them said it wasnt required only a TM30 however i notice the form asks for one...... Does anyone know if I need one for change of visa as maybe she thought it was for an extention.

Additionally for item 3 do they really want every page of my passport or just the Bio page and my Visa exempt stamp page including the 30 day extension i got yesterday.

You will need PROOF of your residential lease agreement... most offices require one year. Provide a signed and dated copy of owners Thai ๐Ÿชช ๐Ÿ†” or owners passport with copy of the signed lease agreement.

ASK YOUR THAI IMMIGRATION OFFICE for the length of the lease required for the one year extension of stay as retired.
