What documents are required for a non-O marriage visa application in Thailand from the UK?

I require some assistance with some of the required documents need to apply for a Thai visa.

I am British and applying for a visa (non-o marriage) in the UK

The first is residence permit for county in which application is submitted - I have uploaded my passport again as I am a British citizen

Letter from spouse saying we are married still and they acknowledge the visit, is there a set format or will a short letter be enough.

Medical insurance for covid is this needed for the visa I am applying for? I have health insurance from my company but it doesn't explicitly say covid

Document to verify my current residency - utility bill or bank statement?

2. No set format. Just have her write one in Thai

3. Not needed. But you have to upload something so either write N/A and scan & upload or use your existing insurance.

4. Utility bill or drivers license should be fine.
