Can anyone tell me what document i need to bring to immigration for the covid visa extension. Do I need a picture (which format)?

It should be extended until December 31, however Bangkok and a couple of immigration offices are getting very strict about issuing those extension of stays
I have NOT seen anyone be formally denied a 60 day extension of stay based on unable to travel due to COVID-19 yet (as in, they got the 7 day extension of stay denied stamp).
I have seen dozens of reports of people at Changwattana, Nonthaburi, and other locations with lots of tourists being verbally rejected. In most cases, the immigration officer will issue the extension of stay IF they show a valid flight out of Thailand within 60 days.

Different immigration offices have different requirements, but all require the following:
1. Your passport and:
a. a copy of your data ("picture") page
b. a copy of your most recent visa (if you have one)
c. a copy of your most recent stamp
d. a copy of your departure card
2. [May not be asked at Bangkok Immigration] A TM30, filed in the immigration office.
3. The forms (each immigration office may have different forms, and want to use their special forms). The application form is called a TM. 7
4. A letter from your embassy saying you can not return, OR a print out from your embassy website saying they don’t issue the letters, OR a print out of a letter from the embassy saying they don't issue those letters. SEE the "OR's"
5. 2x passport pictures
6. 1,900 THB.
Different immigration offices have different policies. *SOME* may require:
- The ID card of the property owner where you are staying
- Copies of *ALL* stamped pages in your passport
There are lots of reports recently of verbal rejections at Changwattana if you already have ONE 60 day extension of stay based on unable to travel due to COVID-19

Hi I've checked the TM30. It seems to be a NOTIFICATION filled by the landlord. So I don't know how I could fill it myself. Could you please explain to me?

It's a passport sized photo, but everything can be obtained at their office for a small fee. A few pages of your passport will need to be photocopied but it's easy to just do it there. They supply all forms to fill out as well.
