TLDR : Answer Summary
To apply for a Covid-19 Visa extension at the Mung Thong Thani immigration office, you will need the following documentation: a copy of your passport data page, a passport-sized photo, proof of filing a TM30, a copy of your departure card (TM6), a copy of your latest entry stamp, a filled out TM7 application form, a filled out STM2 acknowledging understanding of overstaying rules, and a completed form indicating your intention to remain in Thailand due to Covid. The total fee for the application is 1,900 baht. Most immigration offices can provide the necessary forms on-site.

A copy of your passport data page, a passport photo, proof of filing a TM30, a copy of your departure card (TM6) a copy of your latest entry stamp, a filled out TM7 (application form for extensions), a filled out STM2 (acknowledgement that you understand the rules for overstaying) and a filled out “I don’t want to leave because of Covid form” - I have no idea if it has an official name or number. And 1,900 baht. The forms are available at your local office. Most have places on site or nearby that can do the copies and photos.

The usual application form and covid extension form with copies of the relevant pages of passport and proof of address and passport sized photo. My local immigration charge me 100 bht for filling out the forms for me. Easy process.
