TLDR : Answer Summary
To obtain a 60-day extension Covid visa in Thailand, you will typically need the following documents: 1. Your passport and copies of the data ('picture') page, the most recent visa, the most recent stamp, and the departure card (TM6). 2. A TM30 form filed at the immigration office. 3. Relevant forms specific to the immigration office. 4. A letter from your embassy stating you cannot return, or proof that your embassy does not provide such letters. 5. Two passport-sized photos. 6. A fee of 1,900 THB. Note that requirements can vary by immigration office; some may not require certain documents, such as the embassy letter.
Thank you for your answers ! I am currently in Bangkok. Which immigration office do you recommend me to go ?

If you use Jomtien immigration don't need letter or print out from Embassy, just write in the form that embassy not give letter, and don't need TM 6 (departure card) done already many times
this is my experience at Jomtien immigration, I did already many times the 60 days covid extension from last year, easy, not much documents, no documents from embassy, no tm 6 if you lost, I think they care only 1900 THB and eventually a tip
i think the other reason in this covid19 situation government needs revenue for paying immigrations monthly. That is why 1900 baht is very nice money in this covid19 situation.
good then thanks I got stamp from there more than 25 times in 10 years. Some officers look very angry there. Some of them very friendly.I will try try thank you.
