What Documents Are Required for COVID-19 Emergency Visa Extension in Koh Samui?

Does anybody know which documents exactly are required for the covid 19 emergency visa extension on Koh Samui?

How many copies and photos?

Embassy letter printed from email is ok?

Do we need any kind of proof of accommodation? If so, how to do this if it is a private house (not a resort, hotel, hostel etc.) Which is rented from a local Thai?

TLDR : Answer Summary
To obtain a COVID-19 emergency visa extension in Koh Samui, you will need multiple copies of various documents: typically 3 copies of the application form, embassy letter, and passport, along with around 3-4 passport photos. It is suggested to also provide proof of accommodation, such as a rental agreement and a TM30 form, which can be printed out. Additionally, include copies of your ID page from your passport, the owner’s ID card, and the blue book if applicable. Ensure to bring sufficient funds for any potential fines related to accommodation reporting.

print out a form called TM30, attach a copy of your id page of your passport, copy id card front back of house owner, copy blue book house owner and bring enough money to pay the fines for not reporting a foreigner at the address within 24 hours after arrival.

3 copies of application form, 3 copies of embassy letter, 3 copies of passport, 3 or 4 passport photos. Normally there is an office outside koh samui immigration that helps you to complete required paperwork, dont know if they are there now though
