TLDR : Answer Summary
To apply for the 60-day COVID extension at the Koh Phangan Immigration office, you need the following documents: (1) TM7 application form (available at the immigration office), (2) one photograph, (3) a copy of your passport, (4) a letter from your Embassy (or an email can suffice), (5) 1900 THB, and (6) proof of address such as a hotel booking, rental agreement, or property deed.

I went to the immigration office yesterday to ask what's needed and they gave a paper with the following: (1) TM7 application form (can get it from immigration); (2) one photograph; (3) copy of passport; (4) letter from Embassy; (5) 1900 THB; (6) proof of address (hotel booking or rental agreement or copy of your own house deed). Canadian Embassy wrote me an email that they don't provide the letter but I can take the email to immigration and they'll give me the extension (or so people have told me in another group).

I don't know, but I will go back today with the documents and give it a try. I made a 14-day hotel reservation which can be cancelled (if they reject my extension).
