What documents are required for a 60-day Covid extension in Phuket and are there any insurance requirements?

I know the requirements seem to change for the 60 day Covid extension depending on the immigration office. My husband and I have already got our 30 day extension and will be going to get our 60 day Covid extension this week. Has anyone been asked to show Covid insurance or any other documents apart from the TM7 and TM30? Thank you 😊 we are in Phuket!

(HAT YAI) I went today for 1st 60 day covid extension. I went yesterday for 30 Day Tourist Visa Extension. Both times they gave me all of the forms I needed. I just needed copies of the passport pages, 1 passport photo, and 1900 baht.

Specific to Hat Yai, I was told to come 5 days after my tourist visa expires for my "under consideration date" appointment. Then they told me I would get 55 days after that. I have not read that about other offices, so it seemed a little different.

yes.. They told me I was registered as living in Bangkok. I told them I registered the place I was staying as phuket after ASQ. They told me I need to go to phuket to do my extension. I told them I was staying in Hat Yai now, and they made me go to another building and register an address in Hat Yai for me to be able to extend my visa at Hat Yai.

I edited my answer above to show that they now require copies in portrait mode only and copies of any previous Covid extensions.

just make sure you sign every page. All the documents required can be found at the office. No Covid insurance required, notification of residence receipt is required though

A copy of your passport data page (signed),

A passport photo,

Proof of filing a TM30,

A copy of your departure card (TM6) (signed)

A copy of your latest entry stamp (signed)

A filled out TM7 (application form for extensions) - the form doesn’t have a space for your email or phone number but just write it on the back (page 2).

A filled out STM2 (acknowledgement that you understand the rules for overstaying)

A filled out “I don’t want to leave because of Covid form” - I have no idea if it has an official name or number.

And 1,900 baht.

Edit: Update:

In Phuket they require the copies to be in portrait not landscape. This means the top of your passport aligns along the shorter side of the A4. They also want copies of previous Covid stamps (if any)
