Hello! Has anyone successfully applied with invisalign?

Which documents did you exactly provide?

I will be applying through Jakarta. They already got back to my email, confirming that I may apply with Invisalign. But I am unsure which documents are needed. My treatment has already begun and is fully paid. Another 1-2 years of followup appointments are expected.
