What do I need to take to the Koh Samui immigration office to extend my Australian exempt visa?

Hi I am in Koh Samui atm on an Australian exempt visa and wish to extend my stay. I have looked at other comments and tried to do a google search as to what I need to do exactly. I know I need to go to the immigration office and apply for this but what do I need to take with me?

Also at what point do I need to apply for this?

I have been here for a week only, is it too early to apply now?

Do I need a return flight booked also?

Samui accepts extension applications within the last 30 days (technically, you can apply anytime now). But, If I remember right, sometimes (not always) they tell you to come back in the last week for tourism extensions.
