I applied for my second extension today. Everything went smoothly thanks to the kind and helpful officers at Jomtien Immigration. After accepting my documents, they gave me a form stating I need to bring an updated bank statement/bankbook to the same counter along with my next 90-day report. I'm aware of the financial requirements to keep the extension valid, but I don't remember receiving this document after my first extension last year — seems like a new requirement to me.

The problem can appear if the day you must go to show your updated bank account you aren't in Jomtien. I suppose the solution is, case by case, solved at Inmigration office
very well could be the reason. OP wasn't clear at all if he was an 800k or 65K kinda guy. I should have deduced as you did since he did say it was his first.
you cannot get the one year extension of stay as retired in the first year. Thai Immigration offices require 800,000 Thai baht in a Thai bank account in your name only for 2 full months before you can apply for the one year extension of stay as retired
