Thank you, everyone, but we are not going to list all provinces' requirements unknowingly where the OP is gonna apply for her visa.
The Phuket list wouldn't help that much the applications in Surat Thani.
And even people up the requirements that they used for the application, you still need to get the latest one by yourself before you apply.

This is the list from the Phuket Office. My father had an issue in that his rental agreement was signed by a person with Power of Attorney. The officer insisted that as well as the owner’s id and house book, we also needed to supply the power of attorney document and the individual who signed the lease on behalf of the owner’s id and his house book.

My advice would also be not to leave it to the last minute (15 days before your current stay expires). With such an extensive list, there is a high probability that something won’t be in order.

It's not 90 days extension. It's an in-country Non-O VISA based on retirement/being over 50. Those are different.
You NEED to go to your local immigration office to get the latest requirements handout. Those are immigration offices specific. There is no one-fit-to-all list. And those could be updated anytime.
Here is the General information, but this is NOT the same as the one from any of the provinces.
