Hi can any one tell me which form to download for a retirement visa I have got a 3 month o visa and need to change to a retirement visa thanks

Presuming your Non-O was based on "Retirement", then as

And you're not changing to any type of visa. You're applying for an extension of your existing non-O visa based on retirement.
