Picked up the Re-Entry Permit form a few days before.
Filled out form and took with me signed copies of Passport pages, Identity page, TM6 departure card, Entry stamp into Thailand and Non O Extension retirement permitted until date ../../…. stamp and one passport sized photo.
Received queue number at Information desk and was in and out within 5 minutes.
