You can stay 365 but if you don't leave you just have to do an annual check-in

yes, if you stay for one consecutive year you’ll need do a year report, the same way as others do 90 days report.

I think you're not understanding the question. Some visas for some countries require that the person spend at least a certain number of days in the country each year in order to keep the visa valid. For the LTR visas, there's no such requirement. You could never enter Thailand for 4 years and the visa would remain valid for when you enter.

that’s part of the visa policy

So you might as well return get a checkup and a prescription filled, massage, etc to feel vindicated in paying for the health insurance

If you dont use it its still valid and you can still extend. And if its a 3, 5 years etc. you font have to come each year
