Yes, an Extension of Stay based on the Non Immigrant O-A visa (be aware the -A) needs health insurance.

Is it true that you don't need medical insurance for the non O visa? If so, why this discrepancy?

O visa is category "other" and can be applied to multiple reasons for stay, whereas OA visa is geared to retirement. I accept my interpretation might be misguided and the mods will berate and beat me to a pulp for disseminating misleading info.

No requirement of medical insurance for Non-O. They are different visas, different requirements.
