Is Chiang Mai a city/town or province for immigration forms?

Is Chiang Mai considered a city/town or province? Or both? For example I have an immigration form for a Canadian application and it has a space for City/Town and one for Province.

Would it be Chiang Mai, Chiang Mai? Or Chiang Mai City, Chiang Mai?

I'm always confused by this. Sometimes I've used A. Muang as the city but I don't know if that's right.
