I am about to go to MMT, Bangkok to do my first 90 report. I have everything I need but one webpage says a passport photo is required too. There’s no place for it to be glued to the tm47… is it actually required? Don’t want to have to go get one taken if it’s not actually needed. Thanks.

You don't need any copies of anything. you need your passport, that little paper they gave you when you got your extension telling you when your 90 day report was due (IF you have one) and a filled out TM47.
You have to use the same address that you used when you filed a TM30 (and you should have the TM30 receipt in your passport)
which is good IF the person posting was in Mae Hong Son, but it looks like they mean to say they were going to go to MTT and instead typed MMT :O but at least they said bangkok,
