Sorry, I shoulda been more clear. I have a TM30. Phuket immigration has never asked me for it when I did an extension in the past. I’ve done it 2 or 3 times now.
by Thai law, your Address of where you are staying MUST be reported to Thai Immigration within 24 hours of your arrival into Thailand... at the address where you will be staying.
If you stay in a hotel, The hotel will report your stay... that's why they ask to see your passport. If staying in a condo or Air BnB the owner is responsible for filing of the TM 30. Many do not as they want to stay in silence and not have to report income to Thai tax officials. In that case you can report yourself... ask your Thai Immigration office for their instructions.
Without it being filed ... you will NOT 🚫 get the 30 day extension of stay.

That totally depends on the office where you want to apply. Most offices accept a tourism extension application when you have 30 days or less with an entry stamp. Some offices such as Bangkok, Chiang Mai, Phuket and a few more allow you to apply when your stamp remains 45 days or less. But there are a few offices that let you do it only within the last week or two of your stamp such as Jomtien, Samui.
Go to check your local immigration office for their policy.
