How do I successfully use the TM30 app for notifying my return to Thailand?

Update on the online TM30 notifocation. I just came back to the country and used the TM30 app to notify, notification was successful but could not get the screenshot after notifying had to log via website and do a search of notification there, everything has recorded. Note that you can only search notifications on the website that are less then 7 days old, so screengrab.

Note for people who will use the app, point of entry choices are all in Thai only
