Does my 90-day reporting period restart with a new non-B visa, and how do I report it without the small paper?

Dear all,

I have some questions to ask you.

i have got my new non b visa on feb 4 visa as my previous one was not able to do extension. Since i went out of thailand on sept 24 for tourist visa i did not report my 90 days.

So i got confused whether my 90 days report starts new or have to follow the previous one. And also i realized now that i did not get that small paper attached with new non b. So how am i going to report 90 days? Please advice me. Thank you
