Do I need to submit a form for a foreign insurance company if my policy is with a Thai insurance company?

To Admin... My OP stated the policy I acquired is with a THAI insurance company which is included on the list of approved THAI insurance companies list. Question; Does Thai immigration (division 1) need this form for a foreign insurance company even though my policy is with a THAI insurance company.

- The original post (OP) was declined. The OP is mute in that my question is clarified above. American, non OA ,retired, extension of stay. The insurance company is Aetna Health Insurance (Thailand) Public Company Limited.

I still have the old policy however it does not conform with the required coverage. I have already contracted the new policy because I believed the insurance company must be on the immigration approved list which my other policy was not. Because of the dates of my extension, I was not required to provide proof of insurance last year. With my new policy, I have, in hand, a "Certificate of Insurance", "Receipt of Payment" and a letter of coverage for Covid-19. Again I ask, do I need the new insurance company to also fill out the form which is intended for a "foreign insurance company?

if its a Thai insurer on the approved list...NO ...the overseas insurance cert is used only when making OA application in home country and you are using an insurer not on the list.

no, they don't. A Thai insurance company from the approved list, does not have to sign the "foreign insurance certificate"
