Regarding health insurance required by immigration for renewal of Non immigrant OA visa, does it have to be one of the 12 providers listed at longstay.tgia.org or can our insurance provider (not listed) complete the form as it complies with the requirements of 40,000 outpatient 400,000 inpatient.

You can't renew an OA visa. Are you talking about an extension from an immigration office (in which case you must use one of the Thai recommended insurers) or a new visa from an embassy (in which case you can use your own insurer with the form completed)?
