I am visiting the USA. I have an OA visa. I was vaccinated and me my second shot is 6 July. I need to apply for a COE. I had a return flight scheduled for 24 July but I need to return 10 July. On the COE form am I required declare and provide documentation that am vaccinated? I am unsure if the COE can be done in time.

I do not think they can complete a COE in 4 days anyways

If you're coming in via ASQ in Bangkok, there's no vaccination required. Not sure why they make it so difficult to bypass that portion of the COE application but you're not required to be vaccinated to do ASQ so try submitting without it.

If you wanted to do the Phuket sandbox, you won't qualify because you have to have received your 2nd dose 14 days before the time you arrive.

Just keep in mind that for the thai government to accept your vaccine status you have to have the second dose 14 + days before arriving. If you are going through Phuket this rule applies. Depending on which vaccine you received they can do the 2nd shot 3 weeks prior to the 1st. Still can't figure out why it takes 14 days to be effect...
