Do Americans applying for the DTV visa in the USA need to provide a flight ticket and intended accommodation details?

For Americans that applied for the DTV in the USA, did you have to show a plane ticket and intended place to stay ? The LA consulate lists neither as a requirement and it says you can use the visa whenever you want. 
But did they make you show a plane ticket and place to stay in Thailand or no ?

How months of bank statements did they want ?

Because when you apply it says don’t apply more than 3 months before intend stay.

Washington DC, asked for flight number but not a purchased ticket. Yes, they asked for an address where you will be staying.

Put your intended date within 3 months, they may or may not want to see your ticket, I'm betting they won't. You might book a hotel for a week if they request to see accomodation and say you're looking for longer term on arrival.

I’m applying to the LA consulate this week and am adding my rental lease agreement, my plane ticket back to Thailand and tax return statements from 2023. They don’t list these as requirements but I know a few people who were asked for flight details + rental lease agreement (or just accommodation in general)

I emailed the LA consulate and asked specifically about tax returns and they told me it’s a requirement now and needs to be included in my application.

I’m providing one month PDF bank account statement and balance verification letter from my bank.
