In Chiang Mai.  If you leave via domestic flight/ terminal to catch a separate international flight at bkk. 

Can you get re entry visa at chiang mai as it is past the international security bit.

The counter is BEFORE the security bit on the second floor in the international departures building. And yes you can buy it there anytime. Bring money and pictures.
