1. Med certificate within what time frame of applying for OA

2. Medical certificate by USA. Doctor

3. Copy of 6 months checking savings showing monthly funds , need bank guarantee ,

4 police report is state local of Federal required?

Email the thai consulate in Los Angeles and ask if you can apply for an O-A visa there from New York.

And as far as the New York questions you have my question to you IS

Did you go to the Thai consulate in NYC and read the requirements for the O-A visa on their website?

I ask that because the questions you're asking ARE answered on the website.

New York WILL accept a medical certificate signed by a thai doctor as long as you use their form from the website.
