What is the application process and requirements for a Non-Immigrant O-A (M) Visa for a U.S. resident?

Getting definitive info for a friend... He is a resident of California, USA. He will be applying by mail for a Non Imm O-A (M) Visa. He will have the 800K + in his bank account for 3 months at time of application. Finer points needed: easiest way to get "police" background cert. (or proper term). Same for medical. What form used? He can find a doctor. Funds - Bank documents needed? Noterized by whom? Very recent Bank Statement I suppose. Insurance: I downloaded the form from here - but he may opt for Thailand based health insurance - opinions on this. Other things / info / advice? And yes we will check out the LA Thai Consulate on this.

Question is...will Thai Immigration upon each entry into Thailand be wanting to see the certificate of coverage above to check to see if your policy dates are valid for the date stamp you are eligible for ..???

And the policy must be valid for the time period of the O-A visa and its extended time with Multi-entry use.

and it will be interesting to see how easy it is to get it filled in and signed. Typical Thai vs USA policies... Much different words to describe a policy.

I will be in contact this week with my insurance provider in US. And with my retiree pension benefits service unit to find way to expedite ... even get the form completed. Also note they want ending date of policy coverage. As a retiree of a major insurance company I have lifetime worldwide coverage and no maximum annual nor lifetime benefit.

Yes - Thank You. My friend and I have a Thai friend that may be able to get a Thai Insurance Policy - If it proves to be a better option.

And if the friend already has coverage which meets the mininum required for the O-A ...there is a form which can be submitted which to prove coverage.

Note that as of today there is no mention of health insurance being required on the Los Angeles website. However the effective date of that new requirement is October 31. I would be watching the website for updates on Oct 31+.

Required Documents: (Required 3 sets; 1 original and 2 copy)

Your actual Passport or Travel Document and one copy of your Passport. (Passport or Travel Document must be valid for at least 18 months and contain at least ONE completely empty visa page).

Visa application form completely filled out (black and blue ink only) (Download)

Addition Application form (Download)

Medical certificate (must be notarized) showing no prohibitive diseases as indicated in the Ministerial Regulation No.14(B.E. 2535) certificate shall be valid for not more than three months (Download)

Three photo’s passport-size photographs (2″x2″) (photocopy or photo taken from Photostat will not be

accepted). Photographs must have a light color background with a full- face view of the person without wearing a hat or dark glasses. Photos must be taken within 6 months.

Bank statement or evidence of adequate finance showing a deposit of the amount equal to and not less

than 800,000 Baht or an income certificate (an original copy) with a monthly income of not less than

65,000 Baht, or a deposit account plus a monthly income totaling not less than 800,000 Baht In the case

of submitting a bank statement, a letter of guarantee from the bank (an original copy) is required

Letter of verification stating that the applicant has no criminal record (verification have to valid for not more than three months and must be issued from a state or Federal Bureau of Investigation only. Online criminal record without authorizer’s signature is unacceptable )

Copy of applicant’s airline tickets showing flight to Thailand.

In the case where the accompanying spouse is not eligible to apply for the Category ‘O-A’ (Long Stay) visa, he or she will be considered for temporary stay under Category ‘O’ visa. A marriage certificate must be provided as evidence
