Just wanted to update for any interested in the group. I am American and intend to  apply for the O-A at the LA Consulate on Monday. After 3 weeks of waiting, I finally received the signed Foreign Health Certificate from BCBS. I will post the outcome here when I return in 3 weeks. Thanks to all for the advice. Fingers crossed all goes well...
