Am I disqualified for forgetting a past visa application when submitting my DTV?
I submitted my DTV and said no the question of if I ever applied for Visa before.
Apparently I do during Covid 5 years ago and I completely forgot about that occasion. Am I screwed?

Shouldn't really make a difference. I doubt that's a reason they would disqualify for you. But the Thai embassy in Paris is notoriously difficult.
