Hi, in one post i read something about Certificate of Entry and a signed Declaration Form. Do you know where is possible to obtain this documents ? May be in my coutry Thay Consolate or Embassy ?

Obtain from your home country Thai Embassy. And meet the requirements to obtain it. Here is the Thai Foreign Ministry letter.
