it has been a while since i have filled out a visa form. can someone please remind me what to write for the following three things... 'duration of purposed stay' / "Name and Address of Local Guarantor"

"Name and Address of Guarantor in Thailand"

applying for non-o M based on spouse. will duration just be 90 days? i feel like last time for my non-o m i just left that blank.

non-o M based on spouse? Where do you apply and do you mean the 1 year Multiple Entry Non Immigrant O Visa based on being married to a Thai National?

You apply for a 1 year visa visa, so I would say 365 days or as you know your max admitted to stay is 90 days at a time. Name and Address of local Guarantor you leave open and the Name and Address of Guarantor in Thailand is your wife, as you also need her letter to the consulate.
