TLDR : Answer Summary
The user is seeking guidance on how to fill out specific sections of a visa application form for a Non-O M visa based on being married to a Thai national. They inquire about the duration of stay, asking if it should be noted as 90 days and express uncertainty regarding the completion of the form. Comments suggest checking Google for examples and confirm the type of visa being applied for.

You apply for a 1 year visa visa, so I would say 365 days or as you know your max admitted to stay is 90 days at a time. Name and Address of local Guarantor you leave open and the Name and Address of Guarantor in Thailand is your wife, as you also need her letter to the consulate.
