Looking at TM87 Visa Application form as I'll be on a 30 day visa exempt status. .
It ask the reason for Application. Can I say to apply for an O visa? Or should I use another reason?

The TM87 is the change of visa form. Just filing it means you're changing visa type. You need to put the reason you're changing visa types and what for. Are you changing visa types for retirement? Marriage?
