Once you have a new passport you can get laminated cards made at a Kodak store. You dont need to carry them around here. Good luck.

they make a ID size photo copy of your passport and stamp. Place them back to back. It looks like a driver's license size passport.

show the police report and the new passport when entering Thailand and then visit immigration office to get the old passport annotated in the new one would be my guess but it's just a guess
