Has anyone dealt with the issue of their city not being listed in the drop down menu of the city options for permanent address within the DTV online application process? Aside from writing a cover letter, I'm not sure what else I can do and am concerned of a potential rejection for that reason. I will be applying online via Jakarta.

I very much doubt they would care what address you put down. That's the most basic piece of information. As long as you write your name correctly and all other proofs you are asked to supply are attached, you should be fine.
I did for in Australia we live in the Perth hill's and our suburb isn't listed, so I just selected the shire from the drop down box, I'm still waiting for approval, but they no longer asking me for extra documents etc, so that must have been ok
What do you mean? It might "not" be a city in Thailand. For example, I just found out that Hua Hin is not a city when filling my application. For all practical matters, it is, but on the paper it's a I don't remember what (thambon?) under Prachuap Khiri Khan province.
what is the district, sub-district and province you live in (directly off your address) The city on the DTV is usually set to province NOT city for example Jomtien/Pattaya pick Chonburi as the city) ๐
