What should I do if I don't have a reference person in Thailand while applying for a multiple entry visa, and do my children need individual applications?

Hello everyone and thank you in advance for any feedback. I am a US passport holder, applying for a multiple entry visa through the Thai embassy in Washington DC.

I am in the process of filling out the application and am unsure of the following:

A reference person in Thailand (name, address, phone, email). We do not know anyone in Thailand (yet). Should we leave blank?

I am traveling with children. Will each child require there own application/fee or can they travel under my visa?

Website states a passport must be valid for at least 6 months. My daughter is 3 months old and obviously does not meet this criteria. Will she still be able to obtain a Visa?

TLDR : Answer Summary
A US passport holder is applying for a multiple entry visa at the Thai embassy in Washington DC and has several questions: 1) Does she need to provide a reference person in Thailand if she doesn't know anyone? 2) Do her children need individual visa applications and fees, or can they travel under her visa? 3) Can her infant daughter obtain a visa if her passport is not valid for 6 months?
