TLDR : Answer Summary
To obtain a COVID-19 vaccine certificate in Thailand for international travel, start by checking with the facility where you received your vaccination, as they often can provide the necessary documents. For those with a 13-digit Thailand vaccination ID, the Mor Prom app is recommended for obtaining and printing an International COVID vaccination certificate. Alternatively, one user shared their experience of receiving a vaccine passport at a local health department for a small fee after their booster shot.

When I had my booster shot at a local village hospital I was given a slip of paper to show it had been done. I then took that to the main health department in town. They then supplied me with the yellow vaccine passport at a cost of 50 bht. It was all pretty straight forward.
