What does "original appointment" mean on my 90-day report receipt in Thailand?

Quick question: after waiting more than 30 days, I received the "Receipt of Notification" for my 90-day report submitted by mail. At the bottom of the Receipt was a strip of paper with the wording "Next Time, Please send the original appointment". What is the "original appointment"? The latest Receipt of Notification? Something else? I am mystified.
