What documents do I need to take for a TM28 form at Immigration in Thailand?

I am going to Immigration tomorrow with my girlfriend at Samut Prakan just outside Bangkok. This is for her to do my TM30 as I arrived back from Isaan yesterday. Question is when i'm there i'm going to enquire whether they need me to fill out a TM28 as well. What forms/photoocopies will I need to take with me for the TM28? Thank you

The TM28 confusion is from that b/s article from those wing-nuts in Division 6 (Phuket) and the KhaoSod news article.

Just go file the TM30 and you'll be fine..

i have no idea why everybody now gets over excited by a TM28 form, after an Immigration officer talked about it for use in Phuket. But okay, the usual copies of your passport, id page, visa page, extension of stay page and departure card.
