What documents do I need to prepare for a Visa on Arrival in Thailand as a Maltese citizen?

Hi. I'm a citizen of Malta who will be visiting Thailand in December 2022 - January 2023 for 23 days and therefore with the new rules I can get the Visa on Arrival.

Anyone has a list of what we need to prepare (bring with us) to get the Visa on Arrival once we arrive at airport?

None of us mods have experience of entering VOA so can only conjecture. I believe by applying upfront you save the hassle of queueing to fill out the relevant forms. Would love to get feedback from others with direct experience.
