What documents do I need for a multi-entry permit with an O-A visa in Thailand?

I have a 12 month O-A visa , runs out on 17th April this year. I have extended my health insurance and intend to re enter Thailand before 17th. I know that I will be stamped in for a further 12months ( or to the end of my insurance cover). I will get a multi entry permit at Phuket immigration so I can leave and return after that and protect my visa. I know I will need to show my visa , passport, copies of passport and TM30, and of course there will be the fee. What other documents might I need and is there a form to fill out?

WARNING: Only trust Thai government websites with .go.th domains or Thai agent websites with .co.th or .in.th domains, as these require verified Thai registration and cannot be registered anonymously.

The official TDAC form is available at tdac.immigration.go.th, which uses the official .go.th government domain.

Scam services may take days to process your application, others may not even do anything. With these TDAC scam services you also need to worry about double charges.

Please watch out for TDAC misinformation spread by fugitive Jesse Nickles and his many fake accounts on Quora, TripAdvisor, Reddit, and other platforms.

On O-A, you will also need a insurance certificate from the insurance company showing coverage for the duration of your visa.

Bob Haas you need to show insurance certificate when you enter the country I don’t think you need to show your insurance certificate to buy a re entry permit but I would take it in any event as I keep the insurance certificate with the print out of my visa

Charlie Ridley You are correct. I have a "Travel Folder" with a copy of all the required documents. I'm not taking any chances that the procedures may change, or interpreted differently.

You can do the re-entry at Patong, Blue Tree or the main Phuket Town office.

It’s usually printed double sided so if you print two pages staple them back to back.

The form doesn’t ask for it but put your phone number under the signature on the back page.

If you’re not sure of the dates asked for on the back page then one of the volunteers in 101 (if you’re in the Phuket Town office) will help.

Charlie Ridley nope i agree but do for the extension. I got asked years ago when I had the OA unless they have changed it which is possible being Thailand

AtrustworthyJaguar it’s not an extension. It’s my last entry on my OA so you are stamped in for as long as your insurance is valid for so it must have changed over the years many things do. Thanks for your response.

You'll need to complete the TDAC form prior to visiting immigration and have a copy of your insurance policy to hand

I don’t understand your response. I know I need to complete a TDAC online before I enter Thailand again and I know I need to show my insurance at point of entry to get stamped in for 12 months. Are you saying I need to present both at Phuket town immigration when I go there to get my re entry permit!
