What documents are needed to apply for a DTV visa for muay thai training in Thailand?

Does someone have a sample letter for the DTV, for applying with soft power activities? I am training muay thai, but it is hard to explain to my school what I need. I'm the only foreigner that trains here, and there is a bit of a language barrier. The school is accredited by the muay thai association, but this is the first attempt at DTV visa from there. Thanks in advance for your assistance

I have an appointment in Vientiane next week, so I will find out and update here. I just have a signed and stamped application form from the gym on their letterhead, and a reciept for 10 months training paid. The gym has a custom red stamp they put on everything, along with signature..so it looks pretty official. Some stuff is hand written in Thai. Also I have copies of my trainers credentials and muay thai certifications, and some photos of me training. I can show them videos if want. I'm even printing some Facebook posts where the gym featured me, the Google maps page for the gym, a picture of me at the building. I assume lots of this stuff could be unnecessary, but I wanted to hand them a thick pack of paper... Thailand loves paperwork 😅

What is required for proof of enrollment in a soft power course?

Different embassies require different documents. So far, we’ve been asked for, and now include in a pack of 10 documents:

Sports Authority of Thailand gym license.

Board of Boxing Sport Coach’s license (A license preferred).

Ministry of Education, Department of Skills Development instructor's license.

Owner's ID card.

Certificate of the gym being a LTD Company and tax liable.

Itinerary of training program.

Curriculum.

Business card or owner's phone number.

Photos and Google Maps link to the gym’s location.
