My “Medical Extension” for being sick with COVID was approved today with 14 day under consideration stamp. 60 days altogether which was surprising.
Documents we gave Chiang Mai Immigration:
1. Medical certificate from doctor signed saying I must quarantine for 10 days.
2. Power of attorney for friend to represent me. Hard to find at first. Link here: [members only]
3. Passport size photo.
4. A large photo we printed out of me in color.
5. Photo copy of my passport.
6. Photo copy of ID of person on power of attorney
6. Tm30
It was approved. They only returned the medical certificate for me to keep. I hope this helps someone.

TLDR : Answer Summary
The user successfully obtained a 60-day Medical Extension in Thailand due to being sick with COVID-19, sharing the documents required for approval, which included a medical certificate, power of attorney, photographs, passport copies, and a TM30 form. The post highlights the approval process at Chiang Mai Immigration and serves as a useful reference for others facing similar situations.
I was wondering that as well. I assume the Officer figured out I was going apply for a COVID extension looking at my previous stamps but being quarantined made it impossible. That is my guess.
