What documents do I need for a 60-day extension at Chaengwattana Immigration in Thailand due to Covid-19?

I went to Chaengwattana Immigration yesterday for my third 60 days extension due to Covid 19. I brought with me a passport photo, passport copies of the information page, last visa stamp, tm30, tm6, embassy letter and filled out 3 forms given at the immi office. It went all good. Last extension expires Dec. 25. I went to Chaengwattana immi on Dec. 23. I was given until March 1, 2022 for my third Covid extension. Just sharing my experience.
